OUR OFFICE
The Office of the Municipal Civil Registrar is in-charge of the registry of the Municipality. Develop plans and strategies which have to go with civil registry programs and projects. Accept all registrable documents and judicial decrees affecting the civil status of persons and transcribe and enter in the appropriate civil registry books. File, keep and preserve in a secure place the book required by law.
OUR SERVICES OFFERED:
APPLICATION FOR MARRIAGE LICENSE
Requirements:
- Certificate of No Marriage (CENOMAR)
- Legal capacity to contract marriage (if applicant is a foreigner)
- Municipal Form No. 92 (Parental consent)
- Certified True/Machine Copy of birth certificate of applicants
- Parent’s consent (if applicant is 18 years old)
- Municipal Form No. 68- Parental Advice (if applicant is 21 years old but below 25 years old
- At least one of the contracting parties must be a resident of Laua-an, Antique
- Posting for ten (10) days
- Releasing of the License on the eleventh day after ten (10) days of posting
- Death Certificate of spouse (if one of the contracting party is a widow or widower)
- Death Certificate of Parents of contracting party (if the applicant is 18 years old and both parents are deceased. The signature of the Legal Guardian (nearest relative) is required

REQUEST FOR CERTIFIED COPIES OF CIVIL REGISTRY DOCUMENTS (Live Births, Deaths, Marriages)
Requirements:
- Filling-up of request form
- Official Receipt from Municipal Treasurer’s Office
- Documentary Stamp
- If no record on file, clients are advised to request his/her birth record from PSA. (Clients can download application form of Live Births, Deaths, Marriages, fill it out and submit copies at the Municipal Civil Registrar’s Office, Laua-an, Antique)

REGISTRATION OF CIVIL REGISTRY DOCUMENTS (Births, Marriages, Deaths and Legal Instruments)
Requirements:
- For the time of registration of Births, Marriages and Deaths
- Four (4) copies of COLB and COD form prepared by the RHU Personnel
- Four (4) copies of COM prepared by solemnizing officer
- For delayed registration of Births
- Three (3) copies of COLB prepared by LCRO Personnel
- At least two (2) documentary evidences (baptismal certificate, voter’s registration record, SSS)
- Joint affidavit
- Marriage contract of parents (if legitimate)
- Marriage contract of requesting party if married
- If illegitimate- affidavit of acknowledgement signed by the parents with their CTC (civil code) and admission of paternity & his CTC (family code)
- Negative result from PSA
- Community Tax Certificate (CTC) of informant
- For delayed registration of Marriage
- Three (3) copies of COM prepared by LCRO personnel
- True copy/certificate of marriage from church or office
